Do I have to fill out my forms ahead of time?
No. Filling them out first saves time at check-in, but you can also fill them out at the hospital.
How do I fill out the forms?
Fill in every field marked with an asterisk. Use dark ink or type, sign and date where it says to, and cross out a mistake with a single line.
Can I do my forms on my phone?
Digital forms may be available by text before your appointment.
How do I pay at my visit?
Payment is due at the time of service unless other arrangements have been made. Credit and debit cards, checks and cash are accepted.
What happens before a telemedicine visit?
You get a text with a link before your appointment. A medical assistant calls to talk about the reason for your visit, and then your provider sends a link to join a video call. Find a stable internet connection and a quiet, private place.
How do I sign in to my patient portal?
After you schedule your first appointment, you will get an email with a link to register.
